• your name, contact details, date of birth and address
• NDIS information, such as your participant number, plan details and plan management type
• health and disability information needed to provide safe, appropriate supports
• details of your representatives, family members, support coordinators or plan managers
• information you give us through our website, by phone or by email.

3. How we collect it

We collect information directly from you or your representative wherever possible. With your consent, we may also collect it from your support coordinator, plan manager, health professionals or the NDIA.

4. Why we collect it

We use your information to:

• respond to enquiries
• plan, deliver and review your supports
• prepare service agreements and invoices, and claim payments
• meet our legal, NDIS and reporting obligations
• improve our services.

5. Sharing your information

We only share your information with your consent, or where the law requires or allows it. For example, we may share it with the NDIA, your plan manager or support coordinator, the NDIS Quality and Safeguards Commission, or emergency services. We do not sell your personal information.

6. Storing and protecting your information

We take reasonable steps to protect your information from misuse, loss and unauthorised access. Only people who need your information to do their job can see it. We keep records for as long as the law requires, then destroy or de-identify them securely.
